CMS finalized a 2.3% hospice payment update for fiscal year 2027 and an aggregate cap of $36,174.75. These national figures are not a provider-specific reimbursement forecast; wage index and other rules still matter.

The final rule makes the hospice election statement addendum mandatory for all beneficiaries electing hospice. The addendum identifies conditions, items, services, or drugs the hospice determines are unrelated to the terminal illness and related conditions—and thus outside the hospice benefit.
CMS also discusses a Service and Spending Variation Index using claims-based measures. The policy reinforces a practical point: coverage decisions should be explained plainly to patients and families, and quality reporting should be treated as part of care accountability. Providers that do not meet required quality-data reporting face a four-percentage-point reduction to the annual update.
