Beyond the discharge order

A patient can have a completed discharge order and still face a fragile transition. The destination, medications, equipment, symptoms, and follow-up should be understood by the patient and the receiving team.

Five checks before the handoff

1. Is the receiving clinician or service identified?
2. Has the medication plan been reconciled and explained?
3. Are equipment and home supports available at the right time?
4. Does the patient know what change should prompt a call or return?
5. Is the care plan aligned with the patient’s goals?

These are practical clinical questions, not a guarantee against readmission or a substitute for payer-specific coverage criteria. CMS’s care-management guidance describes distinct billing requirements for relevant services.
